The Committee considered a report from the Hampshire and Isle of Wight Clinical Commissioning Group and Frimley Clinical Commissioning Group, regarding the development of Integrated Care Systems (ICS) covering the Hampshire population (see Item 6 in the Minute Book).

 

Members heard that the Health and Care Bill requires ICS to be in place by April 2022, to bring together providers and commissioners of NHS services, local authorities and other local partners to plan and improve health and care services to meet the needs of their population. It was highlighted that this included a shift in focus to wellbeing and population health, and tackling inequalities in outcomes, experience and access.

 

Members asked questions for clarification and commented on what they had heard. Members commented that greater communication with district councils was required. Members highlighted concerns about access to GPs. It was acknowledged that primary care was experiencing high demand. Members expressed concern about Hampshire patients in the north of the county being covered by the Frimley ICS and how the integration across local authority borders would work effectively.

 

RESOLVED:

 

The Committee note the update on the development of Integrated Care Systems (ICS) in Hampshire and the Isle of Wight and request a further update in January 2022.

NHS 111 Performance

 

The Committee received a presentation from the Head of Integrated Urgent Care and NHS 111 Services at South Central Ambulance Service NHS Foundation Trust, regarding the performance of the NHS 111 service in the Hampshire area over the past year (see Item 6 in the Minute Book). It was noted that this item had been deferred from the previous meeting. Members heard that the 111 service had experienced an increase in demand during the pandemic and had recruited additional staff to respond to this.

 

Members asked questions for clarification and heard that:

·         There was a separate call centre being used for booking covid vaccinations

·         The local clinical assessment service includes mental health staff so that those ringing 111 with mental health issues could be appropriately assessed and referred on

 

RESOLVED:

 

That the Committee:

1.    Note the briefing on NHS 111 performance.

2.    Request a written only update later in the year.

 

CCG Merger Update

 

The Committee received a written update regarding the merger of a number of the Clinical Commissioning Groups covering the Hampshire and Isle of Wight area (see Item 6 in the Minute Book). The Chief Executive of the Hampshire and Isle of Wight Partnership of CCGs gave a verbal summary of the paper. It was highlighted that the government had recently published a White Paper on health and care, regarding plans for further join up between health and care services in England. Consideration would need to be given to the implications of this locally.

 

Members commented on the proposed merger and expressed concern regarding wasting money on reorganisation, and wanting to ensure the new arrangements would retain the good work that had taken place at a local level.

 

RESOLVED:

 

That the Committee:

1.    Note the update on the merger of Clinical Commissioning Groups in Hampshire.

2.    Request an update in Autumn 2021 on the development of an Integrated Care System (ICS) for Hampshire and the Isle of Wight.

a.    Hampshire Hospitals NHS Foundation Trust – Care Quality Commission latest report (published in April 2020)

 

The Committee received the latest Care Quality Commission inspection report for Hampshire Hospitals NHS Foundation Trust (see Item 10a in the Minute Book). The Chief Nurse from the Trust was in attendance to summarise the findings and the response of the Trust to any areas for improvement.

 

Members heard that the previous inspection in 2018 put the Trust at ‘Requires Improvement’ rating overall. The Trust were proud to report that at the most recent inspection undertaken in January 2020 improvement had been found across all key strands and the overall rating was now ‘Good’. One ‘must do’ recommendation had been made relating to medicine management. The Trust was on track with responding to the majority of the recommendations.

 

RESOLVED:

 

That the Health and Adult Social Care Select Committee:

 

1.    Note the outcomes of the most recent inspection.

2.    Request an update on any improvement actions by email.

 

b. Clinical Commissioning Group Reform in Hampshire and the Isle of Wight

 

The Committee received an update from the Chief Executive of the Hampshire Clinical Commissioning Group Partnership (see Item 10 b) in the Minute Book) regarding the intention of six Clinical Commissioning Groups covering the Hampshire area to merge in April 2021.

 

Members heard that the intention was to enable some elements of commissioning to be provided at a larger scale where it was efficient and effective to do so, including across the Hampshire and Isle of Wight footprint that matched the Integrated Care System. However, it was intended to maintain a focus on other areas at a local level. It was also expected that this merger would make it easier to integrate with social care which is provided at a Hampshire wide level by the County Council. However, it was noted that the Portsmouth City Clinical Commissioning Group and North East Hampshire and Farnham Clinical Commissioning Group would not form part of the new arrangement.

 

RESOLVED:

 

That the Health and Adult Social Care Select Committee:

 

1.    Note the plans to merge outlined.

2.    Request a further update in early 2021 with further detail on how these changes impact the development of an Integrated Care System (ICS) for the Hampshire area.

Representatives from University Hospital Southampton provided an update on the action plan, must do items, and range of actions to be completed by April 2020.

 

Members heard that the Trust’s overall rating was “Good”, and areas of improvement are being addressed by working differently and prioritizing patient driven care.  Posts are being filled timely and updates made to the facility.  

 

In response to questions, Members heard:

 

Wait time for consultants is being addressed alongside keeping patients informed so they are aware.

 

Improvements in outpatient care requires more work to be done and expected to be addressed by next year’s inspection.  

 

Infection control remains above national average and assurances are easier for newer estates.

 

Nursing leadership been updated, and exemplary committed teams are in place with the best interests of patients in mind and are being nationally recognized with awards.

 

Members commended the improvements in service provisions for the ophthalmology department.  Monitoring of services continue, and efforts are being made to not shift the bottleneck into surgeries.

 

RESOLVED -

 

That the Health and Adult Social Care Select Committee:  

a. Noted the findings of the latest CQC inspection report for the Trust and the improved rating of “Good”    

b. Noted the approach of the Trust to respond to the findings 

c. Requested a further update in September 2020

 

b. CQC Inspection Report from Southern Health NHS Foundation Trust 

 

Representatives from Southern Health NHS Foundation Trust provided an update regarding their October 2019 CQC inspection and the four core services reviewed.  

 

Members heard:

 

The most recent report reflected positive changes, but the Trust are not complacent and a lot more work remains to be done.  Workstreams are being led by Trust leadership and clinicians with an equality improvement plan in place.  Listening to feedback from service users, patients, and families and with their support, improvements can be made with the efforts of compassionate and passionate staff for better services. 

 

Members noted:

 

While mental health services do not receive adequate services as their physical counterparts, serious issues such as patient notes, infection control, access, etc. should not be on this list.  These must-dos should have already been addressed. 

 

Certain concerns are not systemic but can stem from individual cases on individual wards, but some are issues across the NHS.  In some cases, the errors are administrative, and policies are not updated in a timely manner before inspections.  

 

In response to questions, Members heard:

 

There is a constant process of learning, training, evidence-based practice, and taking feedback seriously.  The Trust follow up with individuals and update processes as needed.  

 

The Trust use a proactive program where to engage with users and community services to listen to people’s views and opinions.
